A Clinical Study on Prinatal Deaths


Abstract
A review was made of all perinatal deaths at the Kyung Hee University Hospital during 2 years and 10 months-period from October " 19, 1971 to August 31, 1974. There were 58 perinatal deaths among 2,360 cases of the total deliveries, excluding fetuses weighing under 999 gm or born before 28 weeks gestation.
The results of the study were as follows;
1) The incidence of stillbirth rate was 13. 7, and neonatal death rate was 11.2 The perinatal mortality rate was 24. 6.
2) Seasonable variation was not found. (x2=3.86)
3) Perinatal deaths were caused by prematurely (41. 0%) respiratory distress & respiratory insufficiency (24.3%), unknown causes (15.4%), congenital anomalies (1 I.4%), infections (6.4%) and birth trauma (1. 3%.
4) Maternal complications of perinatal mortality were unknown causes (38.2%), premature rupture of membranes (15.4%0¢¥, fetal malpresentations (12.2%), abrup¡þtio placentae (10.3%), toxemia of pregnancy (8.5%), syphilis (5.2%), placenta previa (5. 1%) & cord prolapse (5. 10). The mortality rate classified according to the causes of maternal side showed the highest peak in the case of cord-prolapse (60.0%).
5) Perinatal mortality rate in premature infants with gestation from 28 to 37 weeks was 22. 1% (p<0.001)
6) Perinatal mortality rate among infants weighing 1, 0000-2, 499 gm was 18.2% and perinatal mortality rate among infants over 2, 500gm was 1.2%. (p<0.001) 7) 50.3% of perinatal deaths had no experience of antenatal care during pregancy.
8) Classification according to sex: mortality rate of male was 2. 7%, that of female was 2. 2%, indicating nonsignificant difference. (x2-0.083)
9) The relation between parity and perinatal mortality was as follows: 50.0% of mortality rate in group para 5, 50. 0% of mortality rate in group para 7. (p< 0.001)
10) The perinatal mortality rate classified according to the maternal age was 18. 2% in the age group of 21, 50. 0% in the group of 42 and 50. 0% in the group of 45. (p<0.001)
11) The most cases of neonatal death occurred within 1 hour after birth (28. 5 %) and within 24 hours after birth (65.6%).
